Position Overview:

The Licensed Practical Nurse (LPN) plays a vital role in delivering both direct and indirect patient care services that address the psychosocial, physical, and overall aspects of patient well-being. Under the guidance of a Registered Nurse, the LPN demonstrates initiative and accountability in their professional responsibilities.

Key Responsibilities:

  • Act as a key liaison between patients and physicians to ensure high-quality patient care.
  • Collaborate with clinical teams to manage and implement the interdisciplinary care plan.
  • Conduct thorough assessments of patients and their families as necessary.
  • Plan and execute clinical care effectively.
  • Facilitate the delivery of care to patients and their families.
  • Evaluate the effectiveness of clinical services and recommend improvements.
  • Measure vital signs, administer medications, and assist in examinations and procedures as required.
  • Document patient history, primary concerns, medication details, and other relevant information in the electronic medical record (EMR).
  • Manage supply inventory, including ordering and stocking for examination rooms.
  • Coordinate testing, obtain prior authorizations, and manage referrals.
  • Schedule patient appointments and surgical procedures as needed.
  • Perform additional tasks as assigned.
  • Familiarity with electronic medical records is highly preferred (experience with Athena is a plus).
  • Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook, is preferred.

Qualifications:

  • High School Diploma or equivalent is required.
  • Graduation from an accredited Practical Nursing Program is mandatory.
  • A current LPN license is required.
  • Previous nursing experience is preferred.
