Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Ascension. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to ensure the best possible outcomes for our patients.
Responsibilities- Provide direct patient care in accordance with established policies and procedures.
- Implement and monitor patient care plans to ensure effective treatment and recovery.
- Monitor and record patient condition, communicating with physicians and other healthcare professionals as needed.
- Serve as a primary coordinator of patient care, working with other disciplines to ensure well-coordinated care.
- Note and carry out physician and nursing orders, ensuring accurate and timely implementation.
- Assess and coordinate patient discharge planning needs, working with the healthcare team to ensure a smooth transition.
- Licensure: Advanced Life Support certification within 6 months of hire date, American Heart Association or American Red Cross accepted.
- BLS Provider certification within 1 month of hire date, American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing, licensure from the Wisconsin Board of Nursing or current home state license if considered multi-state/Compact State.
- Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
