Lakeshore Community Services, Inc. is seeking a skilled Community Housing Specialist to join our team. As a Housing Support Coordinator, you will play a crucial role in providing housing support services to individuals with mental illness and/or intellectual disabilities.

The estimated salary for this position is $16,690 per year, based on a 40-hour workweek.


About the Job:


This is a full-time position, requiring 40 hours of work per week, with regular working hours from 8:00am to 4:00pm. Some evening hours may be required.


Responsibilities:


  • Assist consumers who are being discharged or diverted from Warren State Hospital or other facilities for the purpose of coordinating affordable housing needs throughout the discharge process.
    • Develop relationships with landlords and assist consumers in identifying housing options and obtaining and maintaining housing.
      • Provide support to consumers in an effort to help maintain community tenure and prevent admission to higher levels of care.
        • Assist and/or instruct consumers in activities of daily living (budgeting, banking, grocery shopping, meal preparation, medication organization, laundry, housekeeping and general maintenance.)
          • Provide instruction and support to consumers learning to utilize community resources including transportation.
            • Assist consumers in developing natural social and community supports.
              • Assist consumers in accessing appointments as indicated by the consumers' service plans.

Requirements:


To be successful in this role, you will need:


  • A High School diploma
  • At least 1 year of experience working with individuals with mental illness and/or intellectual disabilities or a combination of related experience and education, preferably community-based
  • A reliable vehicle and willingness to use it regularly
