About the Role

Community Health Net is seeking an experienced Clinical Telehealth Specialist to join our team as a Patient Care Coordinator. In this role, you will play a critical part in delivering high-quality telehealth services to patients across Pennsylvania.

Key Responsibilities
  • Telehealth Services: Conduct comprehensive virtual patient assessments and evaluations, ensuring accurate collection of medical histories and current symptoms.
  • Patient Engagement: Establish rapport with patients to create a comfortable and supportive telehealth environment, promoting open communication and trust.
  • Collaboration and Coordination: Work closely with healthcare providers to gather relevant medical information for diagnosis and treatment, scheduling and coordinating telehealth appointments as needed.
  • Technical Support: Educate patients on how to access and use telehealth technology, addressing any concerns or questions, and facilitate the start and end of telehealth sessions.
  • Data Management: Maintain detailed and accurate electronic health records (EHR) for telehealth encounters, documenting patient information, assessments, and recommendations.
Qualifications and Requirements
  • Licensure: Valid and active Pennsylvania (PA) RN license.
  • Clinical Experience: Minimum of 3 years of clinical nursing experience.
  • Skills and Abilities: Strong clinical assessment and patient interaction skills, excellent verbal and written communication skills, proficiency in using telehealth technology and video conferencing platforms.
Compensation and Benefits

We offer a competitive salary of $85,000 per year, commensurate with experience. In addition to a comprehensive benefits package, including medical, dental, and vision insurance, paid time off, and professional development opportunities.
