Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will be responsible for ensuring patients receive the highest level of care and support during their hospital stay.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to meet their unique requirements.
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
  • Identify and address potential barriers to discharge, such as social determinants of health and access to community resources.
  • Develop and implement strategies to reduce readmissions and improve patient satisfaction.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of employment.
  • At least three years of experience in acute care or case management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
