About the Role
We are seeking a highly skilled and experienced Senior Case Manager RN to join our team at Professional Case Management. As a Senior Case Manager RN, you will be responsible for providing direct care and case management to assigned clients, while also supervising and mentoring junior staff members.
Key Responsibilities
- Provide direct care and case management to assigned clients, ensuring that their needs are met and their care is coordinated.
- Supervise and mentor junior staff members, providing guidance and support to ensure that they are meeting the needs of clients.
- Collaborate with the regional director and other personnel to identify and correct issues, and to improve services.
- Plan, implement, and evaluate care provided, ensuring that it is of high quality and meets the needs of clients.
- Participate in client care conferences, and coordinate and manage client care as needed.
- Serve as a local on-site clinical resource, providing support to ensure that clients' home care needs are met.
- Assist and collaborate with staffing coordinators to ensure that staffing and scheduling are appropriate, and that personnel are competent and meet the needs of clients.
- Adhere to nursing delegation guidelines, and ensure that Agency policies are followed.
Requirements
- Graduate of a state-approved school of professional registered nursing.
- BSN preferred.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- One (1) year of supervisory and/or case management experience preferred.
- Current CPR certification.
What We Offer
- $42.00 hourly, plus shift differentials where applicable.
