Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support throughout their hospital stay.

Responsibilities
  • Assess patients' discharge planning needs and develop plans to ensure a smooth transition to the next level of care.
  • Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
  • Participate in the utilization management process to ensure patients receive the most appropriate level of care.
  • Act as a liaison between the hospital and post-acute providers to ensure seamless transitions of care.
  • Apply evidence-based practice and scientific knowledge to nursing practice, ensuring that care is delivered based on patients' age-specific needs and clinical needs.
Requirements
  • Graduate of an accredited school of nursing or equivalent education.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
