Advanced Illness Management Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at Curo Health Services. As a Nurse Practitioner, you will play a critical role in providing advanced illness management and palliative care to our patients.
Key Responsibilities- Work collaboratively with our interdisciplinary team to deliver comprehensive care to patients with advanced illnesses.
- Assess patients' needs and develop individualized treatment plans to ensure optimal care and comfort.
- Provide diagnosis, treatment, and follow-up care to patients under the direction of our Palliative Medical Director or Primary Care physician.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in evening and weekend call as required, providing clinically competent and responsive care to patients.
- Master's degree in Nursing from an accredited school of nursing.
- Minimum of two years of experience as a Nurse Practitioner, preferably in Family or Adult Medicine.
- Current license as a registered nurse and Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician, part of the Professional Services Corporation, to supervise work in advanced illness management and palliative care.
- Part B Provider certification and Federal DEA license.
