About the Role

We are seeking a highly skilled and compassionate Advanced Illness Management Nurse Practitioner to join our team at Illumia. As a key member of our palliative care team, you will play a vital role in providing high-quality, patient-centered care to individuals with advanced illnesses.

Key Responsibilities
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of our patients.
  • Conduct thorough assessments of patients' medical histories, symptoms, and treatment plans to identify areas for improvement and develop strategies for optimal care.
  • Prescribe medications, order diagnostic tests, and provide treatments as needed to manage patients' symptoms and improve their quality of life.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and address any concerns or questions.
  • Participate in ongoing education and training to stay up-to-date on the latest advances in palliative care and advance illness management.
  • Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Master's degree in Nursing from an accredited institution.
  • Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
  • Minimum of two years of experience in palliative care or advanced illness management.
  • Strong leadership and interpersonal skills, with the ability to work effectively in a fast-paced, dynamic environment.
  • Excellent communication and documentation skills, with the ability to maintain accurate and detailed records.
  • Ability to work autonomously and as part of a team, with a strong commitment to patient-centered care.
