Job SummaryThe Speech/Language Pathologist will provide high-quality evaluation, treatment, and supervision services to patients with communication and swallowing disorders. This role requires strong relationship skills, excellent communication, and problem-solving abilities. The successful candidate will be passionate about delivering exceptional patient care and promoting a culture of excellence within our organization.
Key Responsibilities- Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
- Develop effective treatment plans that address patient needs and goals.
- Treat patients according to the treatment plan of care, ensuring accurate and timely documentation.
- Communicate with healthcare team members, patients, and families to ensure seamless care coordination.
- Participate in care conferences and interdisciplinary team meetings to share knowledge and best practices.
- Provide in-service training to facility staff on restorative and functional maintenance programs.
- Record evaluations, treatment notes, progress notes, and discharge summaries in a timely and accurate manner.
- Meet or exceed productivity standards established by Concept Rehab.
- Recommend equipment and resources to support improved clinical services.
- Act as a clinical supervisor for affiliating Speech/Language Pathology graduate clinicians.
- Comply with state-specific Speech Pathology licensure standards.
- Master's Degree in Speech-Language Pathology from an accredited institution.
- Certification by the American Speech-Language-Hearing Association (ASHA).
- Licensure in the state(s) where services are provided to practice as a Speech/Language Pathologist.
