Job Summary

We are seeking a skilled Physical Therapist to join our team as a Geriatric Care Specialist. As a Physical Therapist with Fox Rehabilitation, you will have the opportunity to work with a diverse range of patients, providing evidence-based care and support to help them achieve their goals and maintain their independence.

Responsibilities
  • Provide geriatric house calls to older adults in their homes, delivering proactive and evidence-based therapy to help them maintain their mobility and function.
  • Develop and implement individualized care plans, working closely with patients and their families to identify and address their unique needs and goals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest research and best practices in geriatric care, participating in ongoing education and professional development opportunities.
Requirements
  • Valid Physical Therapy license in the state(s) of practice, or eligibility to apply.
  • Degree from an accredited physical therapy program.
  • Basic computer literacy skills.
  • Current CPR certification.
  • Integrity, compassion, and enthusiasm for working with older adults.
