Key Responsibilities:
Team Oversight:
Direct the Chronic Care Management (CCM) team, including Clinical Care Specialists (CCS), ensuring effective collaboration and communication across all levels.
Operational Leadership:
Supervise daily operations of the CCM team, ensuring timely and efficient patient care delivery.
Care Plan Management:
Oversee the development and management of personalized Care Plans for patients with chronic conditions, ensuring they are current and accurately monitored.
Performance Monitoring:
Track key performance indicators (KPIs) such as patient engagement, care plan compliance, and health outcomes to ensure the CCM team achieves set objectives.

Shift Structure:
Monday-Friday, 8-hour shifts (Typically 9am-5pm but can vary)
Requirements:
Must possess an RN, NP, PA, or MD license
Excellent communication and time management skills required
Minimum 5 years of experience in nursing or a related clinical role, with at least 2 years in a leadership or management position.
Strong understanding of Chronic Care Management (CCM), including regulatory requirements and best practices.Experience in managing remote teams and telephonic case management.
Proficiency in healthcare technology, including EHR systems and care coordination tools.
Excellent communication, critical thinking, and problem-solving skills.
Demonstrated ability to lead and motivate a team, fostering a positive and productive work environment.
Autonomous, independent worker with a strong understanding of healthcare staffing and needs.
Physical Requirements:
Must be punctual and adhere to the company's Time and Attendance policy.
Must be able to remain seated for the majority of their shift.
