About the LPN Supervisor Role

At Ontario Center for Rehabilitation and Healthcare, we are seeking a skilled LPN Supervisor to join our team. As a key member of our healthcare team, the LPN Supervisor will be responsible for conducting facility rounds, monitoring assignments of aides, and supervising medication assistance times.

Key Responsibilities:
Conduct facility rounds to ensure the highest level of care for our residents
Monitor assignments of aides to ensure proper work flow and efficiency
Supervise medication assistance times to ensure accurate recording and follow-up
Intervene in resident conflicts and behavior issues to maintain a safe and respectful environment
Facilitate communication to ensure timely addressing of medical and mental health situations
Ensure all aspects of facility incidents and accidents are completed and reported to the EHP Coordinator/Administrator

Requirements:
Must be able to work effectively as a team member
Valid state LPN license and good standing with the State Registry
