We are currently looking for a Case Manager who will take charge of the service planning and transition processes for the youth involved in our programs. Key Responsibilities:

  • Conduct comprehensive intake assessments, periodic re-assessments, and facilitate transition planning.

  • Actively participate in the Multidisciplinary Treatment Team (MDT) to collaborate on care strategies.
  • Implement Evidence-Based Treatment models while adhering to fidelity requirements for documentation and treatment delivery.
  • Represent the program at court hearings, meetings, and legal proceedings involving the youth.

Qualifications:

  • A Bachelor’s degree in Psychology, Social Work, or a related field.
  • A minimum of 2 years of experience working with youth populations.
