Hospice Clinical Liaison Job Summary

We are seeking a highly skilled and compassionate Hospice Clinical Liaison to join our team at Alive Hospice. As a key member of our clinical team, you will be responsible for developing and maintaining relationships with healthcare providers, identifying potential referral sources, and promoting the utilization of hospice services.

Key Responsibilities:
  • Collaborate with clinical leadership and teams to identify and nurture potential referral sources and health-related groups for education and outreach.
  • Develop and maintain professional relationships with established providers to promote the understanding of the hospice benefit.
  • Interact with patients and potential referral sources, adjusting discussions to meet individual needs.
  • Exhibit superior customer service and critical thinking skills at all times.
  • Collaborate with the medical community and patients/families to facilitate and promote eligible hospice admissions.
  • Work with referral partners within the hospital to identify and facilitate admissions of patients eligible for GIP level of care.
  • Manage patient discharge from the hospital and the admission process from the hospital to home, IPU, or long-term care facilities.
  • Coordinate tracking of referral sources, assessment of utilization of Alive Hospice services, and preparation of relationship activity.
  • Manage relationships with contracted skilled nursing facilities and long-term care facilities.
  • Self-direct work with minimal supervision.
  • Coordinate educational presentations and in-services.
  • Maintain referral source spending log.
  • Document daily within the EMR and CRM.
Requirements:
  • Registered Nurse with five years of healthcare experience, preferably in hospice sales and marketing.
  • Current Tennessee or compact RN license.
  • Valid Tennessee driver's license and automobile insurance.
