Job Summary

Grace and Glory Hospice is seeking a compassionate and skilled Home Health Aide to provide personalized care to patients in their homes. As a key member of our interdisciplinary team, you will work under the direction of a registered nurse to deliver high-quality care and support to patients and their families.

Responsibilities
  • Personal Care Services: Provide personal care to patients, including bathing, dressing, toileting, feeding, and other activities of daily living, as outlined in the patient's Plan of Care.
  • Care Documentation: Accurately document care provided to patients in our online system, HospiceMD, in accordance with agency policy.
  • Communication: Report any deviations from the Plan of Care to the Case Manager or designee, and communicate effectively with patients, families, and healthcare team members.
  • Equipment Use: Utilize durable medical equipment as needed to provide patient care.
  • Team Collaboration: Foster a cooperative team environment by participating in staff conferences and contributing to the development of patient care plans.
  • Respite Care: Provide respite care to patients' families and caregivers as needed.
  • Quality Assurance: Participate in Quality Assurance and Performance Improvement (QAPI) activities to ensure high-quality patient care.
Requirements
  • Home Health Aide Certification
  • CPR Certification
  • Physical Form: Signed physical form by a doctor (provided by Grace and Glory Hospice)
  • TB Test and Immunizations: Recent 2-step TB test and immunizations or titers (checks for previous vaccinations)
  • Identification: Two forms of identification (driver's license, social security card, passport)
