Patient Coverage Area: Oakland
Employment Type: Full Time (40 Hours/Week)
Work Schedule: 8:30am-5pm, including rotating weekends and holidays
Compensation: The anticipated pay range is $78.30 per hour, reflecting the potential salary based on various factors at the time of posting.
Role Summary: The Hospice Nurse is responsible for admitting patients, conducting thorough assessments of their physical, psychosocial, and environmental needs, and creating tailored care plans. This position involves collaboration with a multidisciplinary team and includes follow-up visits as necessary.
Key Responsibilities:
- Conduct comprehensive initial assessments of patients.
- Document clinical findings, skilled interventions, and discharge plans.
- Collaborate with the Clinical Team Manager to regularly evaluate and adjust care plans.
- Identify the need for additional services from team members such as Physical Therapists, Occupational Therapists, and Social Workers.
- Provide skilled nursing care during all patient visits, including patient and family education.
- Obtain and document physician orders accurately.
- Supervise Home Health Aides in accordance with regulatory standards.
- Coordinate care with the Clinical Team Manager and other healthcare professionals, documenting significant changes in patient conditions.
- Maintain compliance with established documentation standards and technology use.
- Submit weekly schedules for patient visits and collaborate on scheduling needs.
- Perform re-certifications, transfers, and discharges as required.
- Participate in multidisciplinary patient conferences and quality improvement initiatives.
- Engage in ongoing education and training opportunities.
- Contribute to the development of agency protocols and policies.
- Design and deliver educational materials for home care patients as needed.
Qualifications:
- Active California RN license.
- At least one year of nursing experience is required.
- Current CPR certification.
- Valid California driver's license and automobile insurance.
