We are seeking a highly skilled and compassionate Home Health Patient Care Manager Assistant LPN to join our team at LHC Group. As a key member of our home health team, you will play a vital role in ensuring the continuity of high-quality care to our patients.
Responsibilities:- Assist the Patient Care Manager in coordinating a team of staff to provide quality care to patients.
- Prepare patient lists and necessary documents for weekly case conferences.
- Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
- Assist in clinician documentation review/audits to ensure complete and accurate submission of all documentation by field staff.
- Current LPN licensure in state of practice.
- Current CPR certification.
- Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.
