Care Planning Professional

We are seeking a highly skilled Care Planning Professional to join our team at Butterfield Trail Village. As a member of our interdisciplinary team, you will be responsible for the oversight and coordination of the RAI process, including MDS assessments, care area analyses, and care plan development.

This position requires a comprehensive set of skills and qualifications, including an active, unencumbered nursing license in Arkansas, preferably a registered nurse (RN), MDS certification, and a minimum of two years of experience in MDS assessments. Additionally, candidates should have at least five years of long-term care experience to understand the complexities of resident assessments and care planning.

We offer a competitive salary of $**$85,000 - $95,000 per year**, as well as great benefits such as Medical, Dental, Vision, 401(k), Life Insurance, Health Savings Account, Flexible Spending Account, Paid Time Off, and Discounted meals.

Responsibilities
  • Oversight and coordination of the RAI process
  • Management of MDS assessments, care area analyses, and care plan development
  • Collaboration with an interdisciplinary team to complete MDS assessments, analyze care areas, and develop data-driven care plans
  • Expert resource for the Patient-Driven Payment Model (PDPM)
Requirements
  • An active, unencumbered nursing license in Arkansas
  • A registered nurse (RN) preferred but not required
  • MDS certification with a minimum of two years of experience
  • Long-term care experience of at least five years
