Case Manager - Behavioral Health Services

Elwyn Adult Behavioral Health Services is seeking a compassionate and dedicated Case Manager to join our dynamic Behavioral Health Drop-In Center in Raleigh, NC. As a Case Manager, you will provide critical support, coordination, and resources to help individuals with serious mental illness and co-occurring disorders achieve their recovery goals and live more independently.

Key Responsibilities:
  • Advocate for clients and develop individualized plans to facilitate recovery and empower clients to reach their full potential.
  • Build strong therapeutic relationships with clients, creating a safe and supportive space where they feel comfortable discussing challenges and setting goals.
  • Utilize evidence-based interventions, including the PRISM model and Cognitive Behavioral Therapy (CBT), to deliver effective support to clients.
  • Provide in-vivo care, including tasks like medication monitoring and education, providing psychoeducational activities, and helping clients develop skills for daily living (ADL/IADL), community integration, social interaction, and employment.
Requirements:
  • Bachelor's degree in a Human Services field, such as Social Work, Psychology, Rehabilitative Counseling, or a closely related discipline.
  • At least two years of experience in human services, preferably in case management.
  • Strong understanding of recovery-oriented and person-centered care principles.
  • Ability to establish and maintain effective therapeutic relationships and creatively engage clients in discussions and planning around recovery and personalized goals.
  • Knowledge of prescribed medications used in the program, their effects, and the ability to monitor client self-medication.
  • Ability to manage a caseload effectively and prioritize tasks in a fast-paced environment.
  • Proficient in documentation and record-keeping, with experience in electronic health record (EHR) systems a plus.
