Job SummaryWe are seeking a skilled Behavioral Health Specialist to join our team. As a key member of our care management team, you will be responsible for coordinating services, engaging clients in health promotion activities, and sharing information with care managers.
Key Responsibilities- Perform general outreach, engagement, and follow-up with members to ensure they receive necessary support.
- Coordinate services and appointments to meet the unique needs of each client.
- Engage clients in health promotion activities to promote overall well-being.
- Share information with care managers to inform treatment decisions.
- Make and track referrals to ensure seamless care coordination.
- Participate in case conferences to discuss client progress and develop treatment plans.
- Support care managers in assessing and addressing unmet health-related resource needs.
To be successful in this role, you must have at least 18 years of age, a high school diploma (or equivalent), and two years of paid experience performing similar functions. Additionally, you must have at least one year of paid experience working directly with behavioral/mental health clients.
