CVS Health is seeking a skilled and compassionate Clinical Case Manager RN to join our team. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and improving health outcomes.
Key Responsibilities- Assess, plan, implement, and coordinate case management activities with patients to evaluate their medical needs and facilitate their overall wellness.
- Develop proactive courses of action to address issues presented and enhance short and long-term outcomes, as well as opportunities to enhance a patient's overall wellness through integration.
- Collaborate with healthcare professionals to develop and implement effective care plans that meet the unique needs of each patient.
- Provide education and support to patients and their families to promote healthy behaviors and improve health literacy.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- 3+ years of clinical practice experience.
- Active unrestricted NC Compact RN licensure.
- Ability to travel at least 50% of the time outside of the home/office to various member homes, facilities, or community meetings.
- Previous Case Management or Managed Care experience.
- Previous experience with Home Health.
