Position Overview:
Avalon Health Care is currently in search of a dedicated MDS Coordinator to join our team. This role is essential in ensuring the quality of care for our residents while overseeing the Medicare and Managed Care assessments.
Key Responsibilities:
The MDS Coordinator will be responsible for:
1. Managing all assessments for facility residents as directed by the Resident Assessment Director.
2. Overseeing the services provided during the residents' stay, from admission through to discharge.
3. Ensuring the accurate and timely completion of all OBRA required assessments.
4. Collaborating with the management team to contribute to both financial and clinical objectives.
Qualifications:
To be considered for this position, candidates must:
- Hold a current and active Registered Nurse (RN) license in the state of practice.
- Demonstrate strong interpersonal communication skills.
- Be knowledgeable about the RAI process and Case Management.
- Possess the ability to read, write, and communicate effectively in English.
- Comply with all local health regulations and pass any required post-employment assessments, including drug screening and background checks.
Compensation and Benefits:
We offer a competitive pay range starting at $45.00, with a maximum of $51.58 per hour, based on experience and qualifications. Full-time employees are eligible for a comprehensive benefits package that includes:
