Care Planning Specialist

**About the Role:**
As a volunteer with Mercy Volunteer Corps, you will have the opportunity to make a meaningful impact in the lives of others. You will work closely with our team to provide support and care to those in need.
**Responsibilities:**
* Provide support to patients and families in need of advance care planning
* Collaborate with healthcare professionals to ensure high-quality care
* Participate in quarterly retreats and webinars for personal and professional development
* Demonstrate basic computer literacy and familiarity with Microsoft Office products
**Requirements:**
* Associates Degree (AA)
* Ability to sit for extended periods of time
* Ability to use the telephone as primary mode of communication with patients
* English language proficiency
