Coordinate patient care, continuously assess the needs of patients and their families, and implement the interdisciplinary care plan. Deliver services in accordance with physician orders and the established plan of care (POC). Ensure that care delivery complies with federal and state regulations.
**Qualifications**
- Graduation from an accredited clinical education program.
- Current licensure in the state of practice.
- Minimum of one year of experience as a licensed professional.
- Basic technology skills, including mobile device operation.
- Experience as a licensed professional may be considered with regional and executive service line clinical leadership approval.
- Experience in Medicare home health or hospice is preferred.
- Valid state driver’s license.
- Automobile liability insurance as mandated by law.
- Reliable transportation in good working condition.
- Ability to safely operate a vehicle in various weather conditions.
- CPR certification for healthcare providers.
