Key Responsibilities:- Provide quality patient care and perform technical skilled services in accordance with the plan of care and state laws governing nursing practice.
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues, visit assignments, and schedule changes.
- Document patient visits thoroughly and accurately, maintaining an up-to-date medication listing.
- Participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.
- Attend meetings, including case conferences, and demonstrate a positive attitude, willingness to help patients and the agency, and openness to learning new skills.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
