Job Overview:
The Speech Language Pathologist (SLP) is responsible for assessing, designing, implementing, and documenting therapeutic interventions tailored to the individual needs of patients. This role involves comprehensive evaluation and ongoing documentation of patient progress towards established goals.

The SLP is tasked with creating and documenting a discharge plan, ensuring all necessary paperwork is completed for various service locations, and adhering to the requirements set forth by third-party payers.

Key Responsibilities:

1. Conduct thorough evaluations to develop and implement effective intervention strategies that align with the patient's care plan, focusing on remediation, adaptation, disability prevention, and health promotion.

2. Provide ongoing intervention, evaluation, and documentation of progress, adjusting therapeutic methods as necessary, and collaborating with other therapy team members to meet patient objectives.

3. Manage daily schedules across multiple sites, making quick adjustments to accommodate new patient evaluations or changes in existing schedules.

4. Ensure compliance with third-party payer requirements by maintaining accurate documentation and adhering to insurance guidelines.

5. Formulate and document discharge plans for patients who have met their therapy goals, achieved maximum benefit, or opted to discontinue services.

6. Offer appropriate follow-up resources, services, and training for patients and their families as needed.

7. Foster collaborative relationships with key personnel at all service sites, including administrators, nursing directors, MDS coordinators, dietary managers, and restorative nursing staff.

8. Complete all required documentation punctually and in accordance with established practice guidelines.

9. Conduct pain assessments and take necessary actions following standard operating procedures.

10. Perform additional duties as assigned to meet departmental and organizational needs.

Qualifications:
Certifications required include:
- Speech Language Pathologist (SLP) certification from the relevant state board
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- Basic Life Support (BLS) certification from the American Heart Association
- Valid Driver's License with an acceptable driving record

Work Schedule:
This is a full-time position with typical hours from 8:00 AM to 4:30 PM, Monday through Friday, allowing for flexible start and end times. Weekend and holiday work are not required.

Additional Information:
Travel is necessary, with a gas card provided for transportation.
