Key Responsibilities:
The MDS Nurse is responsible for managing the RAI process and conducting assessments as well as coordinating care plans for assigned residents. Key duties include:
- Completing the MDS, CAA's, and care plans within required timelines.
- Conducting resident assessments through physical evaluations, interviews, and chart reviews.
- Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity personnel to discuss resident care needs.
- Reviewing information from hospitals, consultants, and external agencies to inform assessment and care planning.
- Identifying and initiating significant changes in MDS.
- Preparing for PPS meetings and maintaining MDS assessments according to Medicare schedules, as well as monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Staying updated with the requirements set by the American Association of Nursing Assessment Coordinators (AANAC).
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- AANC certification is advantageous. RAC-CT preferred.
- Familiarity with the Resident Assessment Instrument (RAI) process and the principles of the Prospective Payment System (PPS) is highly desirable.
- Prior experience as an MDS Nurse.
