MDS Nurse

We are seeking an experienced MDS Nurse to join our team at Regency at Westland. As a key member of our care management team, you will be responsible for conducting assessments and care plan coordination for our residents.
Responsibilities- Complete the MDS, CAA's and care plans within regulated time frames.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Review information from hospital, consults, and outside agencies and use such information in the completion of the assessment and care planning.
- Coordinate, identify, and/or initiate significant change MDS.
- Conduct PPS meetings and maintain MDS assessments per Medicare schedule and monitor Medicare days and RUGs utilization.
- Registered Nurse, RN or Licensed Practical Nurse.
- AANC certification a plus. RAC-CT.
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse.
