Responsibilities
You will provide psychosocial support and other social work services to Hospice patients and their families in various settings. This will include developing social work plans of care, making recommendations based on patient/family needs, and serving as a liaison between hospice staff and inpatient/nursing home staff.
As a Clinical Social Worker - Hospice Services, you will be responsible for:
- Providing direct admission to inpatient units or nursing home/hospice residents
- Developing social work plans of care and discharge plans when appropriate
Qualifications
To be successful in this role, you should possess:
- A graduate degree from an accredited School of Social Work
- Minimum of one year experience in Social Work in a healthcare setting
- Knowledge of Word and Excel (preferred)
- New York State MSW licensure (LMSW or LCSW)
- Valid NYS Driver's license required
- Excellent organizational, communication, and listening skills
- Ability to input clinical data proficiently
Estimated Salary Range
$64,000 - $80,000 per year + Sign-On Bonus or Student Loan Assistance
