Job Summary

We are seeking a skilled Registered Nurse to join our team as an AFC Nurse Care Coordinator. The successful candidate will be responsible for advocating for the medical needs of assigned program participants, focusing on optimal health in the Adult Family Care setting.


Key Responsibilities

Assess and coordinate healthcare services for participants
Assess and coordinate ancillary services such as Adult Day Health, Home Health, and other services as needed
Review and supervise personal care services at least every 60 days
Assess participant's health status at least every 60 days
Complete and update all care plans in a timely manner and with Division of Medical Assistance (DMA) regulations
Complete semiannual reports in accordance with DMA regulations
Maintain effective communication with participant's physician to obtain medical information, authorization and treatment
Report all medical and nonmedical leave of absences to the Director
Document medical needs and interventions during each participant visit
Advise the Director of any medical problems that may hinder a person's progress or placement in the Adult Family Care setting


Requirements

Registered Nurse
Experience with disabled/elderly adults in a community setting
Excellent communication skills
Valid Massachusetts drivers license
