AFC Nurse

Job Summary

As an AFC Nurse with the Seven Hills Foundation, you will play a vital role in advocating for the medical needs of assigned program participants in the Adult Family Care setting. Your primary focus will be on ensuring optimal health for all individuals.

Key Responsibilities

Assess and coordinate healthcare services for participants, including Adult Day Health, Home Health, and other necessary services.

Review and supervise personal care services at least every 60 days, ensuring compliance with Division of Medical Assistance (DMA) regulations.

Complete and update all care plans in a timely manner, in accordance with DMA requirements.

Foster effective communication with participants' physicians to obtain medical information, authorization, and all medical and non-medical leave of absences.

Document medical needs and interventions during each participant visit, advising the Director of any medical problems that may hinder a person's progress or placement in the Adult Family Care setting.

Qualifications

Registered Nurse with experience working with disabled/elderly adults in a community setting.

Excellent communication skills and a valid Massachusetts drivers license are required.
