Job Summary

We are seeking a dedicated and compassionate Physical Therapist Assistant to join our team of skilled home health care providers. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.

Responsibilities
  • Assist in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
  • Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the Physical Therapist and submitted according to company policy.
  • Perform routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
  • Instruct the client and family and assist in teaching exercise programs to the patient and family.
  • Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report same to the Physical Therapist.
  • Observe and record activities and findings in the clinical record and report to the Physical Therapist and the physician any adverse reaction to treatment.
  • Consult with the Physical Therapist regarding the outcome of home visits and report physical findings.
  • Instruct other health team personnel including, as appropriate, Home Health Aides, as well as family members with respect to certain phases of physical therapy, so that they may work with the client.
  • Write daily progress notes on patients, and submit according to company policy, and prepare periodic summary reports as indicated.
  • Participate with staff, clients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
  • Confirm, at least weekly, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.
  • Assume other duties deemed appropriate and necessary by the Physical Therapist and as designated by Registered Nurse Case Management and Nursing Supervisor.
Qualifications
  • Graduate of an accredited Physical Therapy Assistant program or equivalent education.
  • Licensed to practice as a PTA in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Minimum of one year's experience in home healthcare preferred.
  • Electronic medical record experience is preferred but not essential.
  • Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
