Job Summary:The Physical Therapist will be responsible for evaluating, planning, conducting, and implementing physical therapy treatment plans in accordance with medical referrals. You will work collaboratively with our interdisciplinary team to provide assessments, direct patient care, and develop treatment plans to restore or maintain patients' functional status.
Key Responsibilities:- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes.
- Deliver physical therapy services to comply with prescribed physician orders.
- Maintain plans of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
- Responsible for discharge planning, including collaborating and educating patients and their caregivers on home exercise plans to follow discharge.
- Use appropriate modalities and procedures correctly, demonstrating knowledge of indications and contraindications.
- Participate in coordination of services with healthcare teams, physicians, nurses, and other caregivers.
- Document according to policy and procedure, requiring minimal supervision or instruction.
- Document effectively, resulting in no technical or clinical denials subsequent to review of documentation by payers.
- Maintain effective communication between staff, healthcare team members, and family.
- Serve as a resource consultant for clinical staff.
- Supervise other personnel (PTA, HHA) as applicable.
- Complete OASIS assessments where allowed by state professional practice and regulation.
- Follow policies and procedures and conform to all applicable patient care standards, licensure regulations, and state laws.
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Current CPR certification.
- NPI number.
- One (1) year of recent experience as a Physical Therapist in an acute care or rehab setting.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have a strong sense of smell and touch.
- Must be able to sufficiently reposition patients and move equipment without assistance.
- Must be able to appropriately respond physically and mentally to emergency situations in the home or outpatient clinic setting.
- Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
