Job Title: SDP Intake Coordinator

At Aveanna Healthcare LLC, we are seeking a highly skilled and customer-focused SDP Intake Coordinator to join our team. As a key member of our intake team, you will play a vital role in ensuring a seamless transition for new participants into our Self-Determination Program.

Key Responsibilities:
  • Provide exceptional customer service to new participants and their circle of support, from initial referral to successful transition into the program.
  • Schedule consultations for each referral upon receipt of the Spending Plan.
  • Assess each Spending Plan document and communicate with participants and their circle of support regarding any limitations or issues encountered.
  • Project potential start dates and maintain clear communication with participants and their circle of support.
  • Collect and maintain all supporting documents, including vendor enrollment forms, MOUs, Acknowledgment Forms, and Spending Plans, until transition date.
  • Collaborate with Enrollment and Authorization teams to ensure smooth transitions and maintain projected start dates.
  • Comprehend service codes and service regulations.
  • Maintain accurate notes in AveannaCare.
  • Support the Aveanna mission and culture by demonstrating our core values: compassion, team integrity, accountability, trust, innovation, compliance, and fun.
  • Assist with general office correspondence and answer all direct correspondence.
  • Answer and transfer calls on the multi-line phone system.
  • Lead presentations on Aveanna's Intake process.
Requirements:
  • Care about the developmentally disabled population.
  • Some college or bachelor's degree is strongly preferred.
  • Minimum of one year of customer service experience within the last four years.
  • Excellent interpersonal skills; self-driven, motivated, and results-oriented.
  • Intermediate computer skills using Microsoft Windows, Word, Excel, and ability to learn Aveanna's proprietary software.
  • Ability to successfully handle multiple tasks simultaneously; strong organizational skills.
  • Thrive in a fast-paced environment.
  • Bilingual in Spanish, Vietnamese, or Chinese preferred.
  • Public Speaking.
Physical Demands:
  • Must be able to speak, write, read, and understand English.
  • Occasional lifting, carrying, pushing, and pulling of 25 pounds.
  • Prolonged sitting, walking, standing, bending, kneeling, reaching, twisting.
Environment:
  • Performs duties in an office environment during agency operating hours.
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
