Rehabilitation Nursing Specialist

The Rehabilitation Nursing Specialist plays a crucial role in aiding patients to achieve and sustain optimal functionality following chronic illnesses or significant injuries.
This position involves supporting patients throughout their recovery journey and devising a personalized rehabilitation plan aimed at fostering independence and enhancing quality of life.
Key Responsibilities:
1. Conduct comprehensive assessments of patients' physical, psychological, sociocultural, and spiritual needs, along with their educational and discharge requirements to establish nursing diagnoses.
2. Execute a tailored care plan by delivering nursing care and education directly or through support staff.
3. Organize and facilitate educational initiatives.
4. Leverage suitable resources to create and implement individualized teaching and discharge strategies for patients and their families.
5. Provide direct nursing care by applying the nursing process to achieve optimal outcomes for patients.
6. Supervise and guide ancillary nursing staff.
7. Exhibit professional judgment, employ problem-solving skills, and delegate tasks appropriately.
8. Collaborate with other members of the interdisciplinary rehabilitation team to coordinate nursing care activities that align with overall rehabilitation goals.
9. Adopt a holistic approach to address patients' medical, vocational, educational, and environmental needs.
10. Facilitate community education initiatives to promote acceptance of individuals with disabilities.
11. Assess the effectiveness of nursing care provided and adjust the care plan as necessary to meet measurable goals and objectives.
Minimum Qualifications:
- Valid Registered Nurse License in the practicing state.
- A minimum of one year of professional nursing experience in a rehabilitation environment is preferred.
- CRRN certification may be mandated based on state or client requirements.
- Adherence to all relevant professional standards of practice.
- Completion of Amergis' Competency program as applicable.
- Current CPR certification, if required.
- TB questionnaire, PPD, or chest x-ray as necessary.
- Current Health certificate as per contract or state regulations.
- Compliance with all federal, state, and local regulations.
- Successful completion of new hire training relevant to the job site.
- Understanding of patient confidentiality and HIPAA regulations.
- Strong communication skills with the ability to effectively exchange information with supervisors, colleagues, and clients; proficiency in English is essential.
- Required computer proficiency.
- Must be at least 18 years old.
