Job Title: Intake Coordinator

As a key member of the Cohen Clinic team, the Intake Coordinator plays a vital role in providing exceptional support to our clients. This position serves as the first point of contact for services, managing all intake calls and appointments, and ensuring a seamless experience for our veterans and their families.

Key Responsibilities:

  • Manage intake calls and appointments, administering necessary paperwork and psychometric screeners.
  • Schedule biopsychosocial assessments with clinical staff and conduct phone and face-to-face screenings and intakes on-site and off-site.
  • Work collaboratively with a multi-disciplinary team for case assignments, consultations, and care coordination.

Requirements:

  • Strong organizational skills and attention to detail.
  • Excellent communication skills and ability to work with multiple external partners.
  • Ability to work in a fast-paced, entrepreneurial environment that is mission-focused, results-driven, and community-oriented.
  • Must be able to receive and maintain a Level II Background clearance and have an acceptable Motor Vehicle Registration driver's license record.
  • Master's Degree in Social Services Field, LCSW or LPC eligible, with 3+ years of post-master's intake coordination and/or case management experience preferred.

Preferred Qualifications:

  • Experience working with the Veteran Military population.
  • Bilingual (English/Spanish) preferred.
