$75,000-$80,000

Key Responsibilities:
- Conduct initial nursing assessments for new admissions.
- Complete, coordinate, and update the comprehensive care plan while ensuring continuity of patient care through collaboration with relevant staff.
- Develop, prepare, and maintain accurate and timely individualized patient care records in accordance with care center policies.
- Document changes and outcomes related to pain and symptom management as necessary.
- Educate families regarding patient conditions, needs, and care.
- Refer to other hospice care disciplines as required, or provide rationale for not doing so.
- Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform additional duties as required.
Qualifications:
- Current RN license, specific to the state(s) of assignment.
- One year of nursing experience preferred; candidates with less experience may be considered for the RN new graduate program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
