Hospice Nurse

Responsibilities- Conduct comprehensive assessments of patients and their families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and performing various medical procedures.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Act as a case manager, coordinating patient care and making decisions regarding their treatment plans.
- Document patient information, including medical history, treatment plans, and progress notes.
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal care outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current RN licensure in good standing.
- At least two years of acute care or long-term care nursing experience within the last five years.
- Experience in community health and/or hospice care preferred.
- CPR certification and valid Nebraska driver's license required.
