Role Summary: The Pediatric Patient Referral Specialist and Marketer is essential in fostering and nurturing relationships with referral sources while promoting our services to ensure a consistent influx of patient referrals. The ideal candidate will be an enthusiastic marketer with a solid foundation in healthcare and exceptional communication abilities.

Key Responsibilities

Marketing and Community Engagement:

  • Contribute to the formulation and execution of marketing strategies aimed at enhancing awareness of our services among pediatric clients and Medicaid recipients.
  • Develop and sustain partnerships with healthcare professionals, hospitals, clinics, and other referral sources.
  • Participate in community outreach initiatives, health fairs, and various marketing events.
  • Create promotional materials, including brochures and digital content.

Patient Referral Coordination:

  • Manage and process patient referrals from diverse sources.
  • Utilize clinical knowledge and payer systems to navigate patient care transitions effectively.
  • Communicate with referral sources to collect essential patient information and ensure a seamless referral experience.
  • Collaborate with the clinical team to arrange initial patient assessments and services.
  • Conduct clinical evaluations of referred clients and guide family members in making informed care decisions.
  • Work alongside hospital case managers to facilitate smooth patient transfers and address operational challenges.
  • Maintain precise records of referrals and follow up with sources as necessary.

Client Relations:

  • Act as the primary liaison for referral sources and new clients, ensuring a high standard of professionalism.
  • Deliver outstanding customer service and address inquiries from clients and their families.
  • Ensure client satisfaction by facilitating smooth transitions into home health services.

Collaboration:

  • Work closely with the clinical team to understand patient needs and coordinate care effectively.
  • Collaborate with management to refine marketing strategies based on feedback from referral sources.
  • Provide insights to enhance service delivery and patient care.
Qualifications
  • Associate's degree in Science or a Bachelor's degree in Marketing, Healthcare Administration, or a related field.
  • Preferred credentials include Registered Respiratory Therapist, Registered Nurse, or Licensed Practical Nurse with clinical marketing experience.
  • A minimum of 2 years in healthcare marketing or patient referral management.
  • Strong understanding of home health care services and the healthcare landscape.
  • Excellent communication and interpersonal skills.
  • Ability to cultivate and maintain relationships with referral sources and community partners.
  • Strong organizational skills and attention to detail.
  • Proficiency in Microsoft Office Suite and various EMR and CRM systems.
