Clinical Liaison Nurse

New England Life Care is seeking a skilled Clinical Liaison Nurse to join our team. As a Clinical Liaison Nurse, you will play a critical role in ensuring the seamless transition of patients from hospital to home infusion therapy. Your expertise will be instrumental in developing and implementing strategies to capture referrals from member hospitals and building relationships with hospital staff for quality coordination of care.
Key Responsibilities- Provide clinical assessment of patients referred for home infusion therapy to determine their capabilities to safely receive services at home.
- Develop and implement strategies to capture all referrals from member hospital systems of care.
- Build relationships with hospital staff for quality coordination of care between hospital and home.
- Communicate results of insurance verification to hospital discharge planners and patients.
- Assess patient needs and abilities, and participate in developing patient-specific training plans and plans of care.
- Facilitate discharge or referral processes by relaying orders and documentation to NELC departments.
- Act as a clinical liaison between NELC and hospital departments, including medical staff, care coordination, social services, pharmacy, and nursing departments.
- Maintain a patient caseload averaging 15-20 patients.
- Document all interactions with patients and healthcare teams in patient records and electronic progress notes.
- Effectively communicate patient requirements and changes in patient status to NELC infusion and reimbursement teams, as well as Home Health Agency staff.
- Associate degree or diploma in Nursing.
- Two years of intravenous therapy experience in hospital or homecare.
- Excellent patient assessment skills.
- RN license in good standing in the state(s) where providing care/services.
- Valid driver's license.
- Excellent driving record.
- BS in Nursing.
- Current BLS Certification.
- Knowledge of HIPAA Privacy and Security Regulations.
- Knowledge of JCAHO accreditation process and overall JCAHO standards.
- Working/practical knowledge of home infusion therapy administration/equipment use.
