Primary Care Specialist

Waterfall Community Health Center seeks a dedicated and compassionate Nurse Practitioner to join our team. As a Primary Care Specialist, you will play a vital role in providing high-quality patient care and promoting the health and well-being of our community.
About the RoleThis is an excellent opportunity for a skilled and experienced Nurse Practitioner to work in a fast-paced clinical setting. You will be responsible for delivering primary care services to patients, including health promotion, counseling, education, assessment, acute/chronic care, and consultation/collaboration with healthcare professionals and community resources.
You will work closely with our multidisciplinary team to provide comprehensive care to our patients. Your expertise and skills will be valued and respected as you contribute to our mission to remove barriers to care and promote the health and well-being of our community.
Responsibilities- Provide primary care services to patients, including health promotion, counseling, education, assessment, acute/chronic care, and consultation/collaboration with healthcare professionals and community resources.
- Work closely with our multidisciplinary team to provide comprehensive care to our patients.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Counsel and educate patients on healthy lifestyle choices and disease prevention strategies.
- Conduct routine physical exams, take medical histories, and order diagnostic tests as needed.
- Prescribe medications and treatments as authorized by state law.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between settings.
- Master's degree in nursing from an accredited school of nursing.
- Current licensure as a Registered Nurse in the State of Oregon.
- National certification as a Nurse Practitioner in a relevant specialty.
- Minimum one year of experience as a Nurse Practitioner or related role.
- Ability to communicate effectively in English, both verbally and in writing.
- A competitive salary range of $124,256 - $143,121 per year, depending on experience.
- Student loan repayment program opportunities, including up to $200 per month towards student loan debt and a $50,000 loan forgiveness program for eligible providers.
