Job SummaryWe are seeking an experienced Advanced Practice Nurse (APRN) to join our team as a Home Health Care Specialist. Responsibilities- Provide primary and specialty care services to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private residences.
- Develop and implement individualized care plans that address patients' physical, emotional, and social needs.
- Conduct thorough assessments, diagnoses, and treatments, using evidence-based practices and staying up-to-date with the latest medical research and guidelines.
- Collaborate with multidisciplinary teams, including caregivers, physicians, and other healthcare professionals, to ensure seamless coordination of patient care.
- Maintain accurate and detailed medical records, adhering to HIPAA and OSHA regulations.
- A Master's degree in Nursing (MSN) from an accredited institution.
- An active state APRN license and National Board Certification.
- Current DEA licensure and at least two years of experience as a nurse practitioner.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Proficiency in electronic medical record (EMR) systems, Microsoft Office applications, and other relevant software.
- A valid driver's license and reliable transportation.
