About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Waterfall Community Health Center. As a primary care provider, you will be responsible for delivering high-quality patient care, promoting health and wellness, and working collaboratively with our multidisciplinary team.

Key Responsibilities
  • Primary Care Provision: Provide comprehensive primary care services to patients of all ages, including health promotion, disease prevention, and management of acute and chronic conditions.
  • Diagnostic and Treatment Services: Conduct thorough physical examinations, order and interpret laboratory tests, and prescribe medications as necessary.
  • Patient Education: Educate patients on healthy lifestyle choices, disease management, and treatment options to empower them to take control of their health.
  • Collaboration and Communication: Work effectively with other healthcare professionals, patients, and families to provide seamless care coordination and ensure positive outcomes.
Requirements and Qualifications
  • Education and Licensure: Master's degree in nursing and licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in Oregon.
  • Certification and Experience: National certification as a Nurse Practitioner and a minimum of one year of experience in primary care or a related field.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work autonomously, and proficiency in electronic medical records and practice management systems.
