Job Summary

AleraCare is seeking a highly skilled and experienced Advanced Practice Registered Nurse (APRN) to join our team as an Infusion Nurse Practitioner. This is a part-time position, working 2-3 days per week, Monday through Friday, 8am-5pm.

Key Responsibilities
  • Provide High-Quality Patient Care
    • Conduct initial patient evaluations and assessments, developing a written plan of care.
    • Provide nursing services to assigned patients, ensuring confidentiality and dignity.
    • Collaborate with physicians and other healthcare professionals to deliver optimal patient care.
  • Manage Patient Records and Documentation
    • Prepare and maintain accurate and up-to-date patient records, including clinical and progress notes.
    • Ensure documentation meets all standards and policies, including findings, interventions, and results.
  • Lead and Participate in Quality Improvement Initiatives
    • Participate in the company's Performance Improvement and Continuous Quality Improvement (CQI) programs.
    • Identify areas for improvement and implement changes to enhance patient care and outcomes.
  • Communicate Effectively
    • Provide patients and families with health education, counseling, and referrals to other healthcare professionals and community resources.
    • Communicate effectively with team members, physicians, and other stakeholders to ensure seamless care coordination.
Requirements
  • Education and Licensure
    • Master's Degree from an accredited Nurse Practitioner Program.
    • Holds a current NP state license in the state of practice.
  • Experience and Skills
    • At least 4-5 years of advanced practice registered nursing (APRN) experience, with ambulatory and infusion nursing experience preferred.
    • Strong interpersonal, time management, and organizational skills.
    • Strong written and verbal communication skills.
    • Experience with electronic medical records (EMR) and computer skills, including word processing and efficient use of the internet and email.
  • Physical Requirements

    • The employee is regularly required to stand, walk, sit, talk, and hear.
    • The employee is required to use their hands to operate vehicles and office equipment.
