Job Title: Advanced Practice Provider (APP) for Sickle Cell Disease Program

About the Role:

We are seeking a highly skilled Clinical Provider to join our team at Eastern Virginia Medical School and Sentara Health. As an Advanced Practice Provider, you will play a crucial role in delivering exceptional outpatient and inpatient clinical care to individuals suffering from sickle cell disease.

Your Key Responsibilities:

  • Independently assess, diagnose, and develop treatment plans for patients with sickle cell disease and those requiring primary care services.
  • Promote health promotion examinations and interventions among our patient population.
  • Conduct medical histories, perform physical examinations, prescribe medications under a collaborative practice agreement, and dispense/deliver them accordingly.
  • Distinguish between normal and abnormal findings, ordering appropriate diagnostic studies as needed.
  • Provide anticipatory guidance and counseling to patients/families.
  • Maintain accurate records of patient assessments and care plans, generating relevant outgoing correspondence when necessary.
  • Familiarize yourself with documentation requirements to ensure proper billing practices.
  • Initiate and coordinate referrals to other healthcare team members or agencies.
  • Serve as a consultative resource to nursing staff.
  • Contribute to the development, utilization, and evaluation of resources aimed at meeting patient/family educational needs.
  • Engage in education activities targeting external agencies, community groups, and other healthcare professionals.
  • Participate in quality assessment/improvement/research initiatives within the division and department, contributing to the achievement of set goals.
  • Monitor the task module of the electronic health record to ensure timely responses.
  • Assume administrative responsibilities and special projects as assigned.
  • Collaborate with colleagues to achieve divisional and departmental objectives.
  • Engage in professional development activities to enhance clinical expertise and nursing knowledge.
  • Pursue opportunities for participation in divisional, community, and professional organizations.
  • Maintain effective working relationships with faculty, staff, residents, and the public.
