Care Coordination Supervisor

Integrated Services for Behavioral Health (ISBH) is a community-focused organization providing behavioral health services to individuals in Southeastern and Central Ohio. As a Care Coordination Supervisor, you will play a vital role in ensuring high-quality care coordination services are delivered to children and families/caregivers from diverse backgrounds.

The position requires overseeing the activities of the care coordination team, providing training, support, and assistance to staff members. The supervisor must maintain a thorough understanding of local community resources and develop working relationships with staff, clients, and community partners.

This leadership role will serve Region I, encompassing Fairfield, Perry, Muskingum, Morgan, Noble, Guernsey, Coshocton, and Washington Counties.

Key Responsibilities:

  • Supervise and oversee the activities of care coordination team members through regular meetings, supervision, and communication.
  • Provide comprehensive training for direct care coordinators in the home and community setting.
  • Promote an ongoing culture of teamwork, recognition, and collaboration among team members.
  • Maintain high-fidelity wraparound training credentials and ensure completion of skill and competency-based training for supervising the delivery of ICC and MCC.
  • Review, provide feedback, and approve all required documentation, including care plans, initial assessments, CANS, outcomes reports, etc.
  • Ensure coordination and adherence to on-call rotation.
  • Participate in ongoing fidelity review and monitoring systems focused on consistent application of system of care principles, adherence to OhioRISE ICC/MCC planning process, and service components.
  • Collaborate with relevant systems and key participants to ensure the highest quality of services are provided, prioritizing family voice and choice.
  • Assess needs in crisis situations and inform response.
  • Respect confidential information and ensure operations meet quality and compliance standards.
  • Evaluate performance of staff members and recommend appropriate action to the manager.
  • Participate in the review and selection of new staff, including completing position requisitions, applicant reviews, and interviews.

Required Skills and Qualifications:

  • Bachelor's or Master's degree in social work, education, counseling, psychology, or a related field.
  • LSW/LPC preferred.
  • Three years of experience providing coordinated supportive services/care coordination services.
  • Previous supervisory experience preferred.
  • Experience working in the behavioral health field, preferably with multi-system youth, or individuals with autism and/or developmental disabilities.
  • Working knowledge of local, state, and federal regulations for services in behavioral health, developmental disabilities, and autism.
  • Knowledge of high-fidelity wraparound services preferred (training provided post-hire).
  • Excellent communication skills, both oral and written, demonstrating professionalism with children, youth, families/caregivers, and colleagues, and partners.
  • Excellent organizational skills with the ability to stay focused and prioritize multiple tasks.
  • Demonstrated clinical knowledge of children with co-occurring developmental disorders and complex behavioral health issues.
  • Experience working within Electronic Health Record systems.
