Hospice Care Specialist

We are seeking a dedicated Medical Social Worker to join our team at Ohio's Hospice, Inc.
About the RoleThe ideal candidate will possess strong clinical expertise and judgment, with excellent communication and interpersonal skills. This is a PRN position, covering 10 counties in SE Ohio, with an on-call/weekend rotation as needed.
Duties and Responsibilities- Provide superior care and services to patients at their end of life journey.
- Assess patient and family needs, utilizing advanced educational background and clinical expertise to identify unique cultural/spiritual beliefs and coping skills.
- Offer a wide range of services to terminally-ill patients and families, including pre-bereavement and anticipatory grief support.
- Licensed Social Worker (LISW or LSW) with the state of Ohio.
- Master's Degree in Social Work strongly preferred.
- One year of social worker experience in a healthcare setting.
- Certification as a CHP-SW or ACHP-SW preferred.
- CPR Certified preferred.
- Computer skills sufficient to properly document services and care.
- Able to drive during daytime, nighttime, or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance coverage.
- Notary designation or intention to complete designation within six months of hire.
This role offers a competitive salary of approximately $**65,000** per annum, plus benefits including health, dental, and vision insurance, short- and long-term disability, life insurance, paid time off, matching retirement plans, tuition reimbursement, mileage reimbursement, and more.
