Job Summary

We are seeking a highly skilled and experienced Nurse Practitioner to join our team as a Float Nurse Practitioner for our PACE Program. Responsibilities
  • Provide primary health care to program participants, including initial and periodic assessments, diagnosis, and treatment of medical conditions.
  • Collaborate with physicians and other healthcare professionals to manage episodic and chronic medical needs of program participants.
  • Develop and implement individualized plans of care for program participants, including diagnostic, therapeutic, and nursing interventions.
  • Conduct home and nursing home visits as needed to provide care and support to program participants.
  • Participate in quality assessment activities, including record reviews, outcome measurement, and review/revision of policies and procedures.
  • Provide education and training to program participants, caregivers, and staff on relevant healthcare topics.
  • Act as a resource to program staff and participate in defining nursing standards and policies.
  • Enhance professional growth and self-development through participation in research activities, educational programs, and current literature.
Requirements
  • Masters degree in Nursing from an accredited Nurse Practitioner program.
  • Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing.
  • National Board Certification as a Geriatric, Adult, or Family Nurse Practitioner.
  • One to three years of Nurse Practitioner experience, with at least one year of experience in geriatric care.
  • Demonstrates a broad knowledge of medical conditions and appropriate interventions.
  • Able to apply the principles, methods, and techniques of professional nursing practice to the care of program participants.
  • Able to establish and maintain effective working relationships with program participants, family caregivers, medical staff, and program staff.
Work Environment

This is a full-time position with a mix of home office and on-site work at our PACE Program locations throughout Massachusetts. The successful candidate will be required to travel to various locations as needed to provide care and support to program participants.
