Job Summary

Lifescape Community Services is seeking a highly skilled Senior Transportation Coordinator to join our team. As a key member of our transportation department, you will be responsible for coordinating transportation services for our clients, ensuring timely and efficient transportation solutions.

Key Responsibilities:

  • Coordinate transportation schedules and communicate with clients and staff
  • Manage transportation database and documentation
  • Provide back-up support for Senior Choice caseworker
  • Coordinate transportation trips with local community sites and Senior buildings
  • Accurately complete and process referrals for clients
  • Provide information and assistance to walk-in or telephone consumers
  • Stay up-to-date on available services and how to access them
  • Accurately complete various forms and applications for consumers
  • Perform home visits to assess consumer needs and complete necessary forms
  • Inform and assist clients in gaining access to available benefits and services
  • Provide timely and accurate follow-up with consumers
  • Work with staff, other agencies, and private individuals to find hard-to-reach clients
  • Maintain timely and accurate documentation of service given to clients
  • Assess consumer needs for services not specifically funded and submit requests for Gap Filling services
  • Participate in activities and events to promote agency services
  • Participate in Information and Assistance activities, including giving presentations on benefits and services
  • Attend training as required by Supervisor or Executive Director
  • Represent the agency professionally in manner, speech, and dress
  • Drive the transportation van as needed

Essential Functions:

  • Must be able to read, write, and understand written and verbal instructions
  • Must be able to operate fax, copy machine, basic office machines, and multiple telephone lines
  • Must be computer literate
  • Must be able to effectively and patiently communicate with consumers, caregivers, staff, and visitors
  • Must be able to interpret consumers or caregivers needs effectively
  • Must be able to concisely and accurately convey those needs to the appropriate agencies
  • Must be able to interpret a variety of instructions furnished in written or oral form from government, benefit, and funding organizations
  • Must be able to travel to and safely enter homes and other locations to perform job-related duties outside of the office environment
  • Must be able to work independently

Minimum Requirements:

  • Must be 21 years of age
  • Bachelor's degree or equivalent experience preferred
  • Must have two years direct service experience and/or equivalent training
  • Must have valid IL driver's license, clean driving record, and own transportation with current insurance coverage
  • Must not have been convicted within 3 years of the date of application of any of the following offenses involving the operation of a motor vehicle: Driving Under the Influence of alcohol and/or drugs (DUI), Reckless Driving, Leaving the Scene of an accident involving death or personal injury, Drag Racing, Manslaughter, Reckless Homicide, Reckless Conduct
  • Must be able to work and communicate effectively with older persons, persons with sight and hearing impairments, and persons with a variety of economic, social, and cultural backgrounds with an emphasis on seniors
  • Must be able to perform essential functions of position
